- Pressure Ulcer (Medicare Never Event)
- Postoperative infection
- Postlaminectomy syndrome
- Hemorrhage complicating a procedure
- Accidental puncture or laceration during a procedure, NEC
- Mechanical complication of device, implant, or graft
- Ventral hernia without mention of obstruction or gangrene
- Hematoma complicating a procedure
- Unspecified adverse effect of drug medicinal and biological substance not elsewhere classified
- Mechanical complication of cardiac device, implant, or graft
Notes: According to a new report published by the Society of Actuaries, the following 10 errors account for approximately 69 percent of total error costs.
