- certifying a Beneficiary as terminally ill for hospice care
- certifying a Beneficiary’s need for diabetic shoes
- establishing, reviewing, and signing an individualized cardiac rehabilitation care plan
- establishing a plan of care for home infusion therapy
- referring a Beneficiary for medical nutrition therapy services
Notes: The nurse practitioners (NPs) and physician assistants (PAs) must meet CMS eligibility requirements. These services ordinarily would need to be provided by a physician.
