  2. Primary care physicians remain overworked, underpaid and in short supply.
  3. Managing the hospital-primary care-specialist conflict.
  4. Care coordination and continuity remains daunting and time consuming for medically complex, co-morbid (MCC) patients.
  5. Care variation is common within regional and local markets.
  6. Excessive end-of-life care intervention.
  7. Limited if any patient engagement in self-care.
  8. Patient utilization of out-of-network specialists is allowed.
  9. Regulatory and legal compliance requirements.
